7. Cedula of Applicant (CTC)/ Government Issued ID
8. Affidavit of Responsibility (Pro-forma to be provided by DENR)

10. Land Registration Authority (LRA) Certification for Candon and Sta. Lucia and old surveys only
11. Zoning Certification that the Lot is within the Residential Zone as classified by the MPDC
12. Approved subdivision or consolidation/ subdivision map (if applicable)
